At The Mary Stevens Hospice, our promise is simple: we will be here when you need us most. That may mean managing symptoms, providing end-of-life care, supporting carers, or offering bereavement support.

Accessing our care begins with a referral - and we will guide you through every step.

Who can refer?

Referrals to our hospice services can be made by:

  • Healthcare professionals such as GPs, consultants, district nurses or hospital teams.
  • Social workers or other members of a care team.
  • Patients themselves, or the people important to them, can also enquire directly with us.

Is a diagnosis needed?

We are registered to provide care for people aged 18 and over with a life-limiting illness, whether that is cancer, a neurological condition, heart failure, respiratory disease or another life-limiting condition. What matters most is how illness is affecting day-to-day life and how we can help.

How do I make a referral?

If you are a healthcare professional:

Please download our referral form, complete it and send it via secure email (details below).

If you are a patient, someone important to them, or a carer:

Please contact us directly to talk through the process with a member of our team.

What happens once a referral is made?

Every referral is carefully reviewed by our patient flow team, who take a holistic approach to understanding each person’s individual needs. We consider medical, emotional, psychological and social factors to ensure that every person receives the most appropriate care and support. Depending on individual needs, this may include:

  • Admission to our In-Patient Unit for symptom management or end-of-life care
  • Support through our Day Services Unit, offering therapies, specialised programs, and clinical guidance.
  • Access to Bereavement Services for people who need ongoing support.

Our goal is to provide compassionate, person-centred care that truly meets each individual where they are.
