Consent to Contact
From our point of view, this is potentially the most important form you’ll ever fill out, as changes in the law mean that every single member of the Hospice family must fill out the form below to show that we have consent to contact you. If you don’t fill out this form, then unfortunately the new law means we will no longer be able to contact you AT ALL – meaning you may miss out on Hospice developments, great events, volunteering opportunities and all the amazing things that happen here at the Hospice.
None of the fields in this form are mandatory (other than the name field), so please don’t feel obligated to give us any more contact information than you feel comfortable with.
